[ENBD] opening 2.4.28

If there are no more bug reports on nbd-2.4.27, I'll open 2.4.28.
A working model should by now be on 

    ftp://oboe.it.uc3m.es/pub/Programs/nbd-2.4.28.tgz

It has support for arbitrary sized remote ioctls. And it even works - 
I added ioctl tests to the nbd-test utility (-i 1:2:3:4:5) to be sure.

Actually hunting down the remaining fd and cdrom ioctls and installing
them is another matter (owww!), but I guess that shortly they'll be
there and that also one could do nbd over nbd as a protocol.

I really need to test compilation under kernel 2.2.20. I guess it needs
some work.
